Community Rehabilitation and Support service

The Edinburgh Community Rehabilitation and Support service is a specialist rehabilitation team.

We offer rehabilitation in the community for people recovering from a stroke. We can also help people who have a physical disability and are living with a long-term neurological condition.

 The team supports people with

  • stroke rehabilitation
  • connecting people to their community
  • work and training advice
  • accessing specialist technology
  • appointments at the Edinburgh Smart Home.

The team is made up of

  • occupational therapists
  • stroke specialist physiotherapists
  • care and support workers
  • a work and training advisor
  • the digital technology team.

We support adults who

  • are aged 16 years old or older
  • live in Edinburgh
  • are registered with an Edinburgh GP.

Your first appointment

One of the team will contact you to introduce themself and arrange a time and place to meet.

They will ask questions to learn about you and what is important to you. Over time, they will work with you to develop a plan. This will help to connect you to the right people at the right time to achieve your goals.

How long the team works with you will depend on your needs and goals.

How to ask for support

You can get in touch with the team direct, or through your GP, a carer or a health and social care worker. Please get in touch for a referral form.
