Medical health questionnaire
To support your wellbeing and allow your manager to review any requests for any workplace adjustments, Onboarding will email you a medical health questionnaire which you need to complete.
- The email will come from admin@fit4jobs.co.uk. If you don't receive it, please check your junk/SPAM email folder.
- The email will contain a username and password. Enter these on the Login screen.
- You can then complete the pre-employment Fit4Jobs health questionnaire.
