Outreach support
Outreach support usually involves having one or more support workers who work with you one-to-one for a certain amount of time each week. This could be two hours or 40 hours, depending on your needs. You can get support to do things in your own home, or in the community, including using public transport.
Below you will find a list of all services. If you only want to see the services relevant to you, choose one of the links under Services for or Services for people with.
